Virginia Rehab Physicians is a medical practice specializing in caring for patients in the post-acute rehabilitation setting. This includes patients admitted to an acute inpatient rehabilitation unit, as well as patients admitted to a subacute/skilled nursing rehabilitation unit. These patients often have devastating neurological/orthopedic/medical injuries or impairments. The goal of the rehabilitation physician is to get these patients to a level of functional independence such that they can return home to their lives. This is done by working closely with the rehabilitation team (nurses, physical/occupational/speech therapists, case managers) to develop a rehabilitation plan. In addition, the rehabilitation physician optimizes the patient medically using medications and procedures such as joint injections to facilitate participation in the therapy program. The rehabilitation physician also communicates with other specialists including internists, cardiologists, pulmonologists, surgeons, and primary care providers to ensure a comprehensive medical plan is developed and that the transition of care from hospital to home is smooth.
